Police say five men were travelling east along the motorway at about 4.30pm (AEDT) yesterday, when a fight broke out near the Mamre Rd exit at St Marys in Sydney's west.
The argument escalated when a man in the back of the white van stabbed another seated in the front passenger seat, injuring him in the chest and arm.
The driver of the van then pulled over, where the fight spilled out onto the roadside.
The 57-year-old victim from Campsie in Sydney's south-west was again stabbed, this time with a spirit level, before others in the van intervened.
The men then fled the scene, with the 33-year-old offender from Auburn left stranded on the side of the M4.
The victim was driven to Auburn Hospital where he was treated for his injuries.
Police say no arrests have been made at this time, though inquiries are ongoing.
Anybody who witnessed the incident has been urged to call Crime Stoppers on 1800 333 000.
